Photo Release Form

Fall 2023 Directory Photos: I grant to Lettered Streets Covenant Church the right to take photographs of me and my family in connection with LSCC events. I authorize Lettered Streets Covenant Church, their assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that Lettered Streets Covenant Church may use such photographs of me and my family without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, social media, and Web content. I have read and understand the above.

Fall 2023 Directory Photos: I grant to Lettered Streets Covenant Church the right to take photographs of me and my family in connection with the above-identified event. I authorize Lettered Streets Covenant Church, their assigns and transferees to copyright, use and publish the same in print and electronically ONLY in the church directory—a booklet which will be printed and distributed within the Lettered Streets Covenant Church congregation and available on a password-protected page on the Lettered Streets Covenant Church website. I have read and understand the above.
